Employers subject to the Health Partnership Program or the Qualified Health Plan
may require selection of a provider from within the Network. All other injured
employees may select a provider of his/her choice. The injured employee may seek
a medical provider listing from his/her supervisor, human resources or designated
employer contact. The employee may select his/her own pharmacy or select a
pharmacy from the Express Scripts pharmacy network. Emergency medical care
does not require preapproval and should be secured at the nearest location.

Ohio law requires employers to obtain workers’ compensation insurance for all
employees.
Ohio employers are either state-fund or self-insured. State-fund
employers pay an insurance premium to BWC.
The bureau then pays
compensation benefits directly to you. Self-insuring employers pay workers’
compensation benefits directly to their employees. If you do not know if your
employer is state-fund or self-insured, ask your employer. If your company is self-
insured, file a workers’ compensation claim with your employer.
