At a public meeting held on October 13,
2011, the Industrial Commission of
Arizona determined that Arizona’s
minimum wage of $7.35 per hour would
be increased by $0.30 to $7.65 for
calendar year 2012. Arizona’s minimum
wage of $7.65 per hour will be higher than
the federal minimum wage of $7.25
through 2012.
The employer has the right to direct an injured employee to a doctor of the employer's
choice for one visit. If the injured employee returns to that physician a second time, the
physician becomes the attending physician. After one visit to the employer's designated
physician, the injured employee may change to a new physician upon prior written
approval from the insurance carrier, the Industrial Commission, or the attending
physician. Specialist referrals are at the direction of the attending physician. The
employee may select his/her own pharmacy or select a pharmacy from the Express
Scripts pharmacy network. Emergency medical care does not require preapproval and
should be secured at the nearest location.
Under A.R.S. § 23-908(B), the Commission is required to establish a schedule of fees to
be charged by physicians and physical therapists or occupational therapists attending
injured employees. The Commission is also required to establish a schedule of fees for
prescription medicines required to treat an injured employee.
For purposes of the Fee Schedule, the term "physician" includes chiropractors and
naturopaths.
Fees for certain products, supplies, and services are not included in the Fee Schedule.
This includes fees charged by nurse practitioners or physician assistants and fees for
ambulance services, durable medical equipment, prosthetics, orthotics, and supplies
when used outside a physician's office. If a product, supply, or services are not
included in the Fee Schedule, then you will not find a code for them in the Fee Schedule
(e.g., codes from Medicare's Healthcare Common Procedure Coding System, HCPCS).
